Texoma Community Center (TCC) is one of 39 Texas Community Centers, which are governmental entities as defined by Title 7 of the Texas Health and Safety Code.
Position Summary: This position performs IDD interest list maintenance,intake/enrollments services and is responsible for coordinating and maintaining eligibility status of each individual through the (DID) Determination of Intellectual Disability process. Position is responsible for data entry, assisting with finding alternate placement as requested, mainly in office face to face meetings, as well as performing other assigned LIDDA duties per the Performance Contract in accordance with the philosophy, goals, and objectives of Texoma Community Center. Participates in fidelity reviews as assigned and operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.
